What is the Departure Declaration?

The form consists of 2 parts:
Arrival section (submitted upon entering Japan)
Departure section (submitted when leaving Japan)
The form to fill out is called 再入国出記録 (Sai-nyūkoku shutsukiroku) in Japanese – which translates to Embarkation card for reentrant in English.
When you arrive at an international airport (such as Narita, Kansai, Chubu, Fukuoka...), before the customs area, you'll easily spot small tables where people are filling out forms. Head over there and pick up a 再入国出記録 form to fill out if you plan to return to Japan in the near future.
