How to Fill Out the Front Side (Side A)
1. Flight Name and Number to Japan:
Clearly write the flight code you used to arrive in Japan, for example: VN310, JL752, ANA123...
You can find this information on your flight ticket.
2. Point of Embarkation
If you're flying from Vietnam, write “VIETNAM” or "ベトナム".
If you transited through another country, write the name of the last country before arriving in Japan.
3. Date of Arrival
Write the date you arrive in Japan in the year/month/day format.
4. Full Name
Write your full name as it appears on your passport, in uppercase letters, without accents.
5. Address in Japan
Write the address where you'll be staying in Japan (hotel, friend's house, or union if you're a trainee).
If you don't have specific details yet, you can write the name and address of the union or receiving company.
6. Contact Telephone Number in Japan
Write the phone number of your accommodation or the union/sponsor's number if you don't have a personal one yet.
7. Occupation
Clearly state your current occupation, for example: “STUDENT” (student), “ENGINEER” (engineer)...
8. Date of Birth
Write your date of birth in the year/month/day format.
9. Passport Number
Accurately write your passport number.
10. Accompanying Persons
Write the number of people traveling with you, divided by age groups: over 20 years old, 6-20 years old, under 6 years old.
If traveling alone, leave blank or write “0”.
11-13. Questions about Goods
These are “Yes/No” questions related to carrying prohibited items, cash over 1 million yen, or separately shipped goods.
If you're not carrying these items, mark “No” for all sections. If yes, mark “Yes” and provide details as required.
Usually, you'll select "No" for all items.

You don't need to fill out this side if all the goods-related questions on side A are marked "No".
