1. Flight Number: 
  • Clearly write the flight code you used to arrive in Japan, for example: VN310, JL752, ANA123...
  • This information can be found on your boarding pass.
2. Point of Embarkation
  • If you are flying from Vietnam (for example), write “VIETNAM” or "ベトナム".
  • If you are transiting through another country, write the name of the last country you departed from before arriving in Japan.
3. Date of Arrival
  • Write the date you arrive in Japan in the Year/Month/Day format.
4. Full Name
  • Write your full name exactly as it appears on your passport, in capital letters, without accents or diacritics.
5. Address in Japan
  • Write the address where you will be staying in Japan (hotel name, acquaintance's house, or union dormitory if you are a technical intern).
  • If you do not have specific details yet, you can write the name and address of the supervising organization or company receiving you.
6. Contact Phone Number in Japan
  • Write the phone number of your accommodation, union, or sponsor if you do not have a personal number yet.
7. Occupation
  • Clearly state your current occupation, for example: “STUDENT”, “ENGINEER”...
8. Date of Birth
  • Write your date of birth in the Year/Month/Day format.
9. Passport Number
  • Write your passport number accurately.
10. Number of Family Members Traveling with You
  • Write the number of people accompanying you, divided by age group: over 20 years old, 6-20 years old, under 6 years old.
  • If traveling alone, leave it blank or write “0”.
11-13. Questions Regarding Goods
  • These are “Yes/No” questions regarding prohibited items, cash exceeding 1 million yen (approx. 6,600 USD), or unaccompanied baggage.
  • If you are not carrying these items, check “No” for all sections. If you are, check “Yes” and provide details as requested.
  • Typically, most travelers will fill in "No" for all items.
14: Signature
  • Sign your name in this section.

You do not need to fill out this side if you answered "No" to all questions regarding goods on Side A.
